Making a claim
Making a claim on your health plan is straightforward. Download the claim form:

Please use this claim form if you are resident of the UK only. Click here if you are resident in the Republic of Ireland and would like a claim form.
Or if you would prefer to receive one by post, call the HSF health plan claims department on 020 7202 1381 and they'll be happy to send you one.
What happens after completing the form?
Once you have completed the claim form send it in to us and we do the rest. If your claim is for professional fees we also need to receive the original receipts for any treatment or optical appliances you have paid for. Once you have done this, you should send the form and receipts back to the appropriate address shown at the top of the claim form.
We aim to have all claims settled promptly, particularly where you have had to pay for professional fees.
